Program Registration Agreement
1. Registration and Placement
Submission of this registration form does not guarantee placement. All registrations are thoroughly reviewed to ensure appropriate group sizes, competitive balance, and participant suitability. JT Hockey reserves the right to decline any registration at its sole discretion, in which case a full refund will be issued.
2. Payment Terms & Methods
To secure a participant’s spot in a program, full payment must be completed using one of the following methods:

Credit Card: Full program payment is required immediately at the time of online registration.

E-Transfer: If selecting e-transfer at checkout, full payment must be sent to [Your Email Address] within 24 hours of submitting the registration form. If payment is not received within this 24-hour window, the registration will be automatically cancelled and the spot released. Please include the participant’s full name and camp division in the e-transfer memo field.
3. Roster Adjustments & Program Cancellations
JT Hockey reserves the right to move participants between groups at any time to ensure appropriate player development, physical safety, and competitive balance. If a suitable group placement cannot be offered by JT Hockey, or if JT Hockey cancels a scheduled program in its entirety, a full refund of all payments will be issued.
4. Withdrawal, Refund, and Transaction Fees Policy
Because ice time commitments, insurance, and coaching staff resources are secured immediately upon registration, all customer-initiated cancellations are strictly subject to the following schedule:

30+ days before program start: Refund of the paid amount, minus a $125 non-refundable administrative fee. For registrations originally paid via credit card, the refund will also be issued net of non-refundable credit card processing fees (approximately 3%).

15–29 days before program start: A 50% refund of the total program fee, or a 100% program credit (minus a $25 administrative fee). Any approved cash refunds for credit card payments will be net of processing fees.

Less than 14 days before program start: No refunds or credits will be issued under any circumstances, except for documented medical reasons.
5. Medical Withdrawals
In the event of an injury or medical condition that completely prevents participation, a pro-rated program credit (minus a $25 administrative fee) may be issued. To qualify for a medical credit, a valid, written medical note from a qualified physician must be submitted to JT Hockey within 7 days of the missed session(s).
6. Credit Card Chargebacks and Disputes
By accepting these terms, you explicitly agree that you will not initiate a credit card chargeback or bank dispute for any completed registration or cancellation fee. Any payment disputes initiated to circumvent this agreed-upon refund policy will be contested using this signed agreement as evidence. Furthermore, the participant and family will be permanently barred from registering for any future JT Hockey programs.
7. Code of Conduct and Zero-Tolerance Policy
JT Hockey enforces a strict zero-tolerance policy for bullying, harassment, discrimination, or abusive behaviour from participants or parents/guardians, whether on the ice, in the dressing rooms, or within the facility. JT Hockey reserves the absolute right to remove any participant from a program immediately for code of conduct violations. In the event of removal under this policy, no refunds or credits will be issued.
8. Liability Waiver and Risk Assumption
Ice hockey involves inherent physical risks, including the risk of serious personal injury, illness, or property damage. Participation is entirely voluntary and undertaken at the participant’s own risk. By submitting this registration, parents/guardians assume full financial and legal responsibility for the participant and hereby release, waive, and hold harmless JT Hockey, its owners, directors, coaches, staff, and independent contractors from any and all liability, claims, or demands arising out of participation.
9. Media Release
Video footage may be captured during sessions for instructional and coaching purposes. Photos or short video clips may also be used by JT Hockey for promotional, website, and marketing use. Families may opt out of promotional media at any time by notifying JT Hockey in writing.
10. Privacy Policy
Participant and family information is collected solely for internal program administration, safety protocols, and direct communications. JT Hockey will never sell, lease, or share your personal data with outside third parties.
